Keep the routine simple

Use the same times, places and prompts wherever possible. A medication list, reminder app or organiser can help, but the system should be easy enough to use every day.

Know what each medicine is for

Keep an up-to-date list of medication names, doses and times. If anything changes after a GP, hospital or pharmacy review, update the list promptly.

If someone needs support

Agree what support is actually wanted. Some people only need reminders; others may need a family member or professional supporter to check that a dose has been taken.

Missed or uncertain doses

Do not automatically double a dose. Follow the medicine-specific instructions or seek advice from a pharmacist, prescriber or appropriate healthcare professional.

Review regularly

Medication needs can change. Periodic reviews with the prescribing team are especially useful where there are multiple medicines, side effects, swallowing difficulties or memory changes.
